- Submit Information:
Fill out the Medical Insurance Claim Form below with your information. Insurance providers require a Doctor’s prescription for your medical
equipment/repair request. Make sure your doctor puts your “Diagnosis Code or DX” on the script.
- File Claim
After you submit your insurance claim form, we submit your claim electronically (EDI) to your insurance on the same day you
submit it. Most insurance companies acknowledge receipt of the claim within 48 hours. If your insurance company needs any additional information to process your claim, we will contact
you.
- Payment
Now we WAIT ON YOUR INSURANCE
COMPANY to decide to approve your claim and tell us how much of your equipment they are going to pay for. We will contact you
immediately with their decision and payment information.

Return Policy:
You may return most new, unopened items which were not special orders within 1 day of delivery for a full
refund. Items should be returned in their original packaging. All returns will be subject to a 15% restocking fee. Special order and customized items may not
be returned. We will not accept returns on merchandise that was opened, used, is missing the
serial number or UPC, or returned more then 1 days after delivery. If you would like to make a return or
exchange, you must contact us by phone or email and receive an RMA number before returning your items. Items returned to us without notification
will not be eligible for a refund or exchange.
